The non-incisional method, often called the buried suture technique, creates the fold by securing the eyelid line with medical-grade sutures placed beneath the skin, without an open incision. When a non-incisional fold loosens, the cause is most often an inappropriate indication or insufficient knot depth and number. When the eyelid is thicker or the patient has significant sagging, we recommend the incisional method instead.

I have heard that non-incisional folds always come undone — is that true?
An inappropriate indication or insufficient knot design is by far the most common cause of a fold loosening. We carefully evaluate from the outset whether the eyelid is genuinely a candidate for the non-incisional approach, and design the knots specifically to hold over time.
Who is a good candidate for the non-incisional method?
Patients with thin, elastic eyelid skin and minimal sagging or excess fat are the best candidates. When sagging is present or the eyelid skin is thicker, the incisional method is generally a more appropriate option.
How long does recovery take?
Because swelling tends to be limited, most patients are able to return to daily activities within two to three days. The fold typically settles into a natural appearance over the following two to four weeks.
Will there be any visible scarring?
Because the procedure is performed through small punctures rather than an incision, visible scarring is minimal.
Can asymmetry be addressed with the non-incisional method?
When the differences in eyelid thickness and levator strength between the two sides are not significant, the knot positions can be designed asymmetrically to compensate. When the differences are more pronounced, the incisional method is generally more effective.
Can it be combined with epicanthoplasty?
