
Early Revision of Ptosis Correction (Retraction and Multiple Folds After a Failed Double-Line Excision)


Pre-op view.
On one side, the crease shows a clear set of multiple folds.
The lower arrow marks the newly created incision; the upper arrow marks the original surgical incision. The original line re-adhered, producing a triple fold.
The eye is also opening too widely.
On the patient's left eye (right side of the video), eyelid retraction has set in alongside the multiple folds.
What we want is no multiple folds at all in the immediate post-op view.
That's the most stable starting point.
The principle of revision surgery is to release the adhesions thoroughly and prevent them from re-forming.
Early revision focuses on balancing the iris exposure and preventing multiple folds; the crease itself is not adjusted.
Six months post-op.
The shape looks good, no multiple folds have returned, and the scarring has settled well.
