Today, after a patient had double eyelid surgery at another clinic,
we would like to show you the case of that patient who had revision surgery at our clinic.
After double eyelid surgery at another clinic, this patient developed mucosal show, a multiple-fold (double-crease) eyelid, and slight ptosis,
and wished to improve these issues,
and also wanted to change the out-fold outline into an in-out fold, so she decided to undergo revision surgery.
Today's post
1) multiple-fold (double-crease) eyelids,
2) eyelash and mucosal show,
3) asymmetric ptosis, and
4) surgery to lower an out-fold double eyelid into an in-out fold,
should be very helpful to those interested in these topics.

This patient came to our clinic after several problems developed following incisional eye-shape (ptosis) correction surgery at another clinic.
Looking at the photos first, there are about five main areas that need improvement.
1) The double eyelid line is excessively high,
2) You can see that a depressed scar was created near the epicanthal fold in an attempt to force an out-fold.
3) And there is mucosal and eyelash show,
4) The left eye in the photo also has a multiple fold and looks slightly sleepy,
5) and the two eyes are asymmetric.
The goals of the surgery were:
1) to lower the line,
2) to change the out-fold outline double eyelid into an in-out fold,
3) to correct the mucosal and eyelash show as much as possible,
4) to resolve the multiple fold, and
5) to make the two eyes as symmetric as possible.
First, to lower the line, we set the line low and released the existing adhesions.
Then, using the scar-preserving high fold correction — commonly called the "two-line" technique — we changed the out-fold outline double eyelid into an in-out fold.

We also corrected the mucosal and eyelash show at the same time, but we did not make this the main focus,
because correcting the mucosal and eyelash show too aggressively can risk functional side effects; so while we do not make it the main focus,
fixing it gently at a low position generally corrects the mucosal and eyelash show for the most part.
For the multiple fold, I personally prefer an approach that avoids fat grafting as much as possible, so with this patient we proceeded without any fat grafting.
The left eye in the photo, which looked sleepy, was found in the operating room to have its eye-opening muscle separated and damaged, so we carefully redid the eye-shape (ptosis) correction.
Eye-shape correction involves carefully dissecting the tissue beneath the eyelid and fixing it lower down; the more this is dissected, the more the orbicularis oculi (the muscle that closes the eye) can be damaged, so proceeding too aggressively can worsen lagophthalmos (incomplete eye closure), which is why it is best to have the surgery done by an experienced surgeon.

We lowered the line and turned the out-fold into an in-out fold.
This is the appearance after correcting the multiple fold, the mucosal/eyelash show, and the left eye in the photo that had looked sleepy.
For the depressed scar as well, we released the adhesions to make the depression less visible.

Today we have introduced the before-and-after case of a patient who had been troubled by a high line, eyelash and mucosal show, a multiple fold, ptosis, a depressed scar, and asymmetry.
During consultations, patients sometimes say, "Will it definitely be corrected? If you give me a firm guarantee, I'll have the surgery."
In surgery there are many variables, and in extreme cases the eye is unlikely to become like one that never had surgery, and individual satisfaction can also vary,
so I tell them, "I cannot give you a firm guarantee, but I will do my very best."
When I do my best with the method I recommend, most patients achieve good results,
