Today, we would like to look into double eyelid revision surgery
with Dr. Choi Dong-il of Gangnam Seoyon Plastic Surgery.
Occasionally, among the patients who come in for double eyelid revision consultations,
some have been given incorrect information,
so today we would like to organize the commonly misunderstood aspects of double eyelid revision surgery
and the questions we are frequently asked!

First, many people ask how many millimeters their double eyelid is.
To start with the conclusion: because every surgeon uses a different measuring method, there is no way to know exactly!
In fact, when surgeons measure the millimeters, some measure the height of the double eyelid from the eyelashes upward,
while others measure from the tarsal plate,
some measure with the skin pulled taut,
and some simply measure as is,
so even for the very same double eyelid, the number of millimeters can come out differently!
For example, for the same double eyelid, Clinic A might say 4 mm, Clinic B 7 mm, and Clinic C 9 mm; the numbers can differ like this.
For this reason, saying that a double eyelid is a certain number of millimeters cannot be considered accurate information.
So when patients come in and occasionally ask about it,
I would like to point out that asking may not carry much meaning.
Second, some people occasionally ask, "Doesn't eye-shape (ptosis) correction cause severe side effects?"
There are indeed people who ask this from time to time.
Of course, there may be cases where someone who should not have had eye-shape correction did so and ran into problems.
However, if someone who looks very sleepy-eyed has only double eyelid surgery without eye-shape correction,
they end up looking quite sleepy and the shape turns out unattractive.
In that case, there would have been no point in having the surgery.
For this reason, those who need eye-shape correction are better off having it.
But performing it indiscriminately when it is not needed can cause problems,
so you must get an accurate diagnosis from an experienced surgeon.
Eye-shape correction is a very well-established procedure that appears in textbooks,
so there is no need to assume that eye-shape correction will inevitably cause problems.
Some people ask whether eye-shape correction is absolutely necessary when undergoing incisional double eyelid revision surgery,
but eye-shape correction is not always required.
This is because there are people who open their eyes wide and well without eye-shape correction.
Such people do not really need eye-shape correction.
However, there are also cases like this:
when we actually open the surgical site to perform an incisional double eyelid revision,
we find that the eye-opening muscle has become separated and there is a problem inside.
Even after seeing that the eye-opening muscle is already separated,
we cannot simply leave it unrepositioned just because the patient said they did not want eye-shape correction.
When such a situation arises,
we naturally have to restore it properly, and that alone can produce the effect of the eyes opening well, just as if eye-shape correction had been done.
So the conclusion is that, since those who open their eyes well do not need eye-shape correction,
it really is a case-by-case matter.
Recently, some people have picked up the idea online that removing skin will cause serious trouble,
and out of a vague fear of skin removal,
some insist that skin should never be removed at all.
Let me explain this point.
First, when performing surgery, leaving as much normal tissue as possible
is of course preferable, but there are also cases where skin removal is necessary.
Occasionally, patients come to us because another clinic removed an unnecessarily large amount of skin, leaving their eyes unable to close.
Removing too much does cause problems,
but I believe that removing an appropriate amount of sagging skin is perfectly fine.
Also, in many cases the eyes not closing is due to causes other than skin removal, which I will explain next.
There are largely three reasons why the eyes may not close after incisional surgery.
The first is that the orbicularis oculi, the muscle that closes the eye, is momentarily anesthetized,
so the eyes may not close right after surgery.
The second is that the orbicularis oculi, likewise the muscle that closes the eye, sustains slight damage during the surgical process,
and when the muscle tone drops, the eyes may not close.
Lastly, if the eye-shape correction was tightened a bit too much, the eyes may not close,
but this mostly settles down over time.
Some people wonder what happens to the sutures when, after surgery by the buried-suture (non-incision) method, a revision is done by the incisional method.
In most cases, the sutures can be removed during an incisional revision.
As we make the incision, if that part is protruding, we pull it out and remove it,
but since we do not forcibly dig around to hunt them down,
one or two sutures, or small fragments, may remain.
Their remaining does not cause any significant problem,
and in rare cases where such a suture is not removed,
the suture may later work its way out through the scar,
but since it comes out easily with a simple pull,
there is no need to worry much; just come in to the clinic.
