Revising Multiple Folds (Triple-Fold Cases)


On her two eyes, one side shows a high crease and the other shows multiple folds.
The iris exposure is slightly different between the two sides as well.
Her right eye (left on photo) reads as sleepy as a result.
Both eyes read somewhat sleepy, but that side is more so.
Some patients who develop multiple folds ask whether fat grafting is required to fix them. Personally, I prefer to address them without grafting; that said, when grafting is genuinely needed, it should be done.
This patient did not have fat grafting.
We performed a mild crease-lowering through what's commonly known as the "double-line excision" (두줄따기), and added ptosis correction to address the sleepy look.

The core of multiple-fold revision is releasing the adhesions thoroughly.
Eight months post-op.
The multiple folds are gone, and so is the sleepy look.
The core of the procedure: release the adhesions thoroughly and keep them from re-forming.
