When someone of an advanced age
goes onto the operating table,
there are surely many extra worries.
A procedure with which the patients themselves are most satisfied,
and with which their children as guardians are pleased too —

First, senior upper blepharoplasty can be thought of, put simply,
as surgery that lifts an eye area which has drooped with age.
That is the easiest way to understand it.

Following the design, an incision is made,
and the wrinkled skin and unnecessary fat
are removed, and then the levator muscle is restored to its
youthful position.
(*In older individuals,
the levator muscle has often become loose
or separated from the tarsal plate,
so it is surgically moved back to its proper position.)
After that, it is sutured along the original double-eyelid line
to create a more youthful eye shape.

Even with age, one cannot give up on looking one's best.
While creating the most flattering eye shape,
it also resolves lash-poking, chafing, and a narrowed field of vision,

So then, what kind of surgery is lower blepharoplasty,
a procedure often performed on older patients?
Lower blepharoplasty is a surgery for those troubled
by bulging under-eyes.

Following the design, an incision is made,
and the unnecessary under-eye skin and the protruding
fat are repositioned, the septum is reinforced,
and the midface is lifted.
During suturing, too, the skin and muscle are
pulled upward to make everything taut.
Here, some of you may have noticed
how this differs from under-eye fat repositioning.
Under-eye fat repositioning goes through the conjunctiva
and repositions only the fat, so there is little scarring
and it is simple, but it cannot lift the midface.

Lower blepharoplasty, on the other hand, not only repositions fat
but also lifts the midface, pulling the wrinkled under-eye area
even more taut.
For this reason, somewhat older patients tend to
prefer lower blepharoplasty over
under-eye fat repositioning.
If drooping eyelids are the concern, upper blepharoplasty;
if bulging under-eyes are the concern, lower blepharoplasty —
that would be the way to go, right?

However, in patients of an advanced age,
the eyelids typically droop while, at the same time,
the under-eyes become bulging in many cases,
so upper and lower blepharoplasty are sometimes performed together.
