The first case is one where a multiple fold developed after incisional double-eyelid surgery.

In the photo, the lower line (blue line) is the double-eyelid line from the previous incisional surgery,

and the upper line (red line) is a line formed by adhesion.

The crease did not form along the part where the surgery had been done;

instead, a crease formed above the incision line, resulting in a multiple fold.

The cause of this kind of multiple-fold double eyelid is that something is strongly adhered inside,

combined with insufficient volume between the eyebrow and the incision line.

As a result, it can even look like a "sausage" double eyelid.

There are 3 key points to the surgery:

(1) reinforcing the upper volume,

(2) releasing the adhesion,

(3) preventing re-adhesion.

We release the adhesion where the upper multiple-fold line forms, keep it from re-adhering, and

reinforce the volume between the eyebrow and the incision line.

The patient said another line seemed to be forming above, and when we checked,

it was not that the triple fold had failed to be corrected;

rather, because the line had been folded for a long time,

a faint crease-like mark remained slightly in that area.

In people with naturally small eyes, opening the eyes means using the forehead for a long time, which leaves deep wrinkles on the forehead,

so even after resolving this with eye-shape correction and no longer using the forehead, the forehead wrinkles still remain.

It can be said to be similar to that.

Since this is right after suture removal at one week, the bruising may make the crease appear a little more pronounced,

but re-adhesion rarely occurs with an experienced surgeon, so there is no need to worry at all.

The second case is one where a multiple fold developed after a failed scar-preserving high fold correction.

This patient had a scar-preserving high fold correction at another clinic,

and a crease formed above the new incision line, where the previous double-eyelid line had been.

This occurred because of adhesion to the existing scar,

arising because the adhesion was insufficiently released or because it re-adhered.

We released the adhesion thoroughly and put in place measures to prevent re-adhesion.

The scar has not yet settled, but it is expected to improve after 6 months.

The third case is one where a multiple fold developed because the double eyelid came undone.

This patient had incisional eye-shape correction several years ago, along with fat repositioning above the eyes,

and said that a multiple fold appeared right after the first surgery, forming a triple fold with several lines.

So she had revision surgery one year after the first surgery, but it did not improve.

In the photo, the lower crease (blue line) is the original incision line,

and an unwanted fold (red line) is folded above the incision line.

This shows the sutures removed at one week after surgery.

Up to one month after surgery, it is a settling process affected by swelling and scarring, so continuous monitoring is needed.

During this process, the eyes may become slightly smaller, the scar may protrude, and the line may rise higher.

A fine line can be seen above the double eyelid,

which is the previously folded line showing slightly like a crease as the swelling subsides,

and it disappears as the swelling goes down and the scar fades.
