This patient presented with bilateral lid ptosis, a slightly sleepy look, and asymmetric creases.

In older patients with sleepy-looking eyes, we generally avoid aggressive ptosis correction.

Opening the eye too widely can leave a startled or unnatural appearance.

The protruding fullness below the eye has also been refined.

For patients with a natural lower-lid roll (애교살, aegyo-sal), we preserve the nerve supply during surgery so the roll can re-form.

Patients without a natural roll won't develop one through surgery.

Patients often wonder whether combining upper and lower blepharoplasty in one session will leave the eyes looking unnatural or extend the swelling timeline.

In our experience, the difference is minimal. Sutures come out at the same five-to-ten-day window in either case.
