A patient I operated on yesterday.
Keep in mind he is still swollen. I will keep updating this post as the recovery progresses.
Pre-op, he had pronounced inferior scleral show, with hooded skin giving him a sleepy, half-closed look.

What I mean by inferior scleral show: in the photo above, three borders of the iris are surrounded by visible white sclera — hence the name.
It can give the face a harsh, off-putting impression. The cartoonist Heo Young-Man even covered it in his manhwa Kkol.
A non-incisional approach is not viable in this kind of case.
First, it cannot address the skin hooding.
Second, the amount of ptosis correction needed is large; doing it non-incisionally produces a strong pulling sensation and a higher complication rate.

Immediately after surgery, in the recovery room.
I will update with new photos at suture removal.
His eyelid position is corrected and the inferior scleral show is reduced.
There is a published mechanism for why scleral show improves with this technique. Happy to walk through it in consultation.
More to follow at suture removal.
