A note on lateral epicanthoplasty reversal.
The previous version of this post had an error, so I am reposting.
When a lateral canthoplasty is overdone, the lateral canthal tendon can be damaged.
Without a normal supporting structure, the lower lid drops at the corner and the canthus takes on an inverted-U shape.
That is the situation that calls for a lateral canthoplasty reversal.

The example above shows mucosal exposure at the lateral lower lid.
It looks as if the corner is drooping.

Like a clothesline that has stretched and sags in the middle, the lower portion descends.
The fix is to re-tighten the line at its anchor point.

One week post-op. The lateral mucosa is now apposed to the lid margin. No further mucosal exposure.
