The core message I wanted to get across was this.

Ptosis correction itself is not what causes the complications. The problems come from repeated incisional revisions and from injury to the orbicularis oculi, which then leads to lagophthalmos and other complications.
In the first photo, at one week post-op, the eye is not closing fully — but at this stage that is usually transient and resolves on its own.

However, when the orbicularis oculi has been damaged, the lagophthalmos becomes permanent.
This patient had clear orbicularis oculi injury.
