When Is the Right Time for Early Revision? (with Lateral and Lower Canthoplasty Revisions)
When is the right time for an early eyelid revision? It's a question I hear often, and one I have presented on at conferences.

The right window is generally within one month (about 30 days), and ideally within the first two weeks.

Most early revisions are done on ptosis correction. The procedure must work through the existing incision line — adding a new incision so soon would isolate the skin between the two lines from its blood supply and risk necrosis.
This patient came to us after surgery at another clinic. Her lateral and lower canthoplasty had been overdone, and she was having difficulty opening her eyes.

Crease correction was inadvisable at this stage, but her eyes were heavily inflamed, uncomfortable, and difficult to open — so addressing those issues was the priority.

We restored the over-aggressive lower canthoplasty as well. Lower canthoplasty is rarely indicated, and it's worth questioning whether the original choice was appropriate.
