Today we'd like to talk about men's non-crease (single-fold) eye-shape correction.
Through actual patient cases, we'll explain how these patients undergo surgery,
how the surgical plan is set, and what kind of results appear once the surgery is done.

First of all, this patient looks very sleepy, doesn't he?
He looks drowsy, and his skin is relatively quite thick,
so if a double eyelid were created, it would be prone to coming undone.
As a result, only about half of the iris is visible right now,
and there is no double-eyelid crease.
So for a patient like this, since the primary goal is to make the eyes larger,
we make the eyes clearer and remove the thick soft tissue.
This includes what people commonly describe as removing fat,
but beyond fat, we also trim away part of the hypertrophied, somewhat bulky tissue called the orbicularis oculi muscle,
and there is also removal of ROOF fat, a fat tissue that is usually difficult to remove; we take that out as well to create eyes that look a bit lighter,
and by creating an inner (hidden) fold and rolling it in, we keep the crease as invisible as possible while
the aim is to make the eyes look clear and defined.
This is how it looks 6 months after surgery.
This patient is similar as well.
His eyelids are extremely thick, and only half of the iris is visible.
For this patient too, while keeping the double-eyelid crease as invisible as possible,
the goal was to make the eyes clear and free of that sleepy look,
and to improve his overall facial impression.
For this patient as well, we performed non-crease eye-shape correction,
keeping the crease as hidden as possible while making the eyes clear,
and the surgery was directed toward removing as much of the thick eyelid tissue as possible.
This is how it looks 2 months after surgery; there is still a little swelling remaining.
This patient was someone who used his forehead muscles a great deal.
Because the eyes don't open well, he relies heavily on his forehead to lift them.
Most people with small eyes like this end up using their forehead a lot,
and once the eyes become larger, the forehead later relaxes and drops down considerably.
This patient also had non-crease eye-shape correction along with epicanthoplasty and lateral canthoplasty,
and after surgery he uses his forehead a bit less like this,
and the eyes become clear like this; but since this is still only one week after surgery,
there is still some swelling present.
Comparatively, if you look before surgery, the area just below the eyebrow appears somewhat sunken,
but this appearance is because the forehead was being lifted,
and once the forehead comes down after surgery,
the area below the eyebrow becomes relatively fuller.
With eye-shape correction, the eyelid volume sometimes increases, but at the same time the forehead is used less,
so as the forehead descends, the sunken look below the eyebrow tends to disappear.
In the case of this patient, on one side the double-eyelid crease looks slightly present like this,
while on the other side there are several layered folds that look somewhat undone,
giving a slightly sleepy impression.
This patient too wanted the crease kept as invisible as possible,
while getting rid of these existing folds.
In such cases, a non-incision approach is used only very rarely,
but in most cases an incision is needed to remove them all cleanly.
This is how it looks about 1 month after surgery.
This patient's eyes are fairly clear,
but he has a very deep and high double-eyelid crease.
In Korea this can come across as a somewhat strong or intense impression,
so the patient wanted the crease hidden as much as possible.
In this case there was no need to make the eyes any more open,
but he wanted surgery aimed at lowering the line so that the crease would be as invisible as possible.

This patient had previously had double-eyelid surgery once at another clinic.
The double eyelid improved the skin sagging,
but for men, a prominently visible crease when the eyes are open doesn't quite suit Korean tastes,
so we set the crease line lower so that it is covered and not visible when the eyes are open —
the scar-preserving high fold correction, the surgery commonly known as the "two-line" technique.
Strictly speaking, "two-line" is not an official term; the proper name is the scar-preserving high fold correction — a line-lowering surgery that preserves the existing scar.
The scar-preserving high fold correction sets a new line below the existing double-eyelid line,
and lowers the line by releasing the existing scars.
